Primary Care for Infants, Children & Adults

Primary care is a combination of health promotion, health maintenance, health counseling, disease prevention, diagnosis and treatment of acute and chronic conditions. According to research studies, those that participate in preventative health care by having screenings and annual exams have better overall health outcomes and fewer hospitalizations. Primary care promotes communication between patients and their health care providers and recognizes the patient as a “partner” in health care.

Acute & Chronic Disease Management

Whether you are having new symptoms such as sore throat, fever or cough, or you are dealing with long-term symptoms from chronic conditions such as arthritis, hypertension, gastric reflux or psoriasis, we can help. We strive to help you live a happy, healthy life by diagnosing, treating and managing a variety of conditions guided by the latest evidence-based medical recommendations.

High Blood Pressure Management

High blood pressure (or hypertension) means that the pressure in your arteries is higher than it should be. If high blood pressure is left untreated, it can lead to dangerous health conditions, including heart attack and stroke. According to the American Heart Association, almost half of the American population age 20 and above has high blood pressure, whether it has been diagnosed or not. Risk factors that increase your chance of having high blood pressure are family history, race/ethnicity, older age, unhealthy diet, smoking, drinking too much alcohol, sleep apnea and stress. High blood pressure does not usually have symptoms, so monitoring is important. If you have already been diagnosed with hypertension, we can help you with you plan to keep your blood pressure in an optimal range and decrease your chances of having complications in the future.

Diabetes Education & Management

Diabetes is a condition which requires careful monitoring and management to prevent serious health complications. The most common form, Type 2 Diabetes, occurs when a person’s body cannot use insulin properly. This causes an increase in blood sugar which affects many organs. According the American Diabetes Association, 1.5 million Americans are diagnosed with diabetes each year. If you have never been screened for diabetes, it’s important to talk to a health care provider to see if you are at risk for diabetes and require screening. If you have already been diagnosed with diabetes, we can guide you toward optimal diabetes management.

Immunizations for Children & Adults

Vaccination can prevent deadly diseases for infants, children, teenagers and adults of all ages. Although several rounds of vaccination are given during childhood, adults also need “booster shots” to protect themselves and other family members from illness. The Center for Disease Control (CDC) reports that in recent years, the incidence of pertussis or “whooping cough” has risen. Whooping cough can be extremely dangerous for babies. The CDC recommends that older children, new mothers, and other adults stay up to date on pertussis vaccination so they can form a “cocoon of protection” around the babies that may be too young to be fully protected by the DTaP vaccine. If you are unsure of which vaccinations you or your family may need, bring in your immunization record and we can help you get up to date.

It is directly associated with several serious health conditions including diabetes, heart disease, stroke, high blood pressure, fatty liver disease and some cancers. We can help you identify factors that may be causing you to gain weight and help you develop a weight loss plan that is individualized for you.
